First Visit

Your initial appointment will consist of an examination and consultation explaining your diagnosis and treatment options. Occasionally, some surgeries can be performed the same day as the consultation, however, a complex medical history or treatment plan will require an evaluation and a second appointment to complete the treatment.

Please bring the following information to your appointment

  • Your surgical referral slip and any X-rays if applicable
  • A list of medications you are presently taking
  • Medical or dental insurance cards
  • Necessary completed forms (This will save time and allow us to help you process any claims.)

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the office visits.

A pre-operative consultation and physical examination is mandatory for all patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink 8 hours prior to your surgery. You will also need someone to drive you home.

Be sure to alert the office staff and doctor if you have a medical condition (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) and any medications that you are currently taking (i.e. heart medications, aspirin, anticoagulant therapy, non prescription medications and herbal remedies).


Please bring all x-rays taken at your doctors or dentists office with you to our office. Please note that sometimes additional x-rays are necessary. If you forgot your x-rays or additional films are necessary, we will take them in our office at an additional charge.
